If you are a new patient or if you are an existing patient but your information has changed, please complete the patient information form and bring with you to your appointment. This will assist us in maintaining an accurate record of both personal and insurance information. Please bring any referrals, insurance cards, and co-payments with you at the time of your visit to ensure accurate submission of any charges incurred. Please make sure that we are a participating provider with your health insurance carrier. Regardless of your insurance, payment remains your responsibility and we do accept cash, credit cards, and personal checks. We are participating providers with Medicare, Blue Cross and Blue Shield, Aetna, and US Healthcare among others.
