ADVANCED SEARCH
You can make an appointment with the physician of your choice by calling the physician's office or by clicking the icon to the right. Every effort will be made to see you as quickly as possible and at your convenience. In some cases, you may be asked if you would like to see your physician at a different location or if you would like to see a different physician in order to schedule your appointment sooner.

If you are a new patient making an appointment for the first time, the office will verify that the physician has a relationship with your insurance provider and that the physician is accepting new patients.

When you come to see the physician for the first time, plan to arrive a little earlier than you typically would in order to complete a questionnaire for your medical record. Some of the questions you should be prepared to answer include:

  • The name of your health insurance provider and the policy number(s)
  • Current employment information and contact numbers
  • The name of medications you are currently taking
  • If you have experienced various medical conditions or if members of your family have ever experienced major medical conditions such as heart disease or cancer

Check with your physician for his or her instructions if you experience sudden illnesses or injuries that you feel may require immediate attention. The on-call physician or the triage nurse will recommend what action you should take. This might include:

  • Following instructions at home for treatment
  • Going to the physician's office for evaluation and treatment
  • Going to the nearest urgent care center or hospital emergency department for evaluation and treatment



INITIAL VISIT
On your initial visit, please bring:

  • Your insurance card
  • Personal identification
  • List of allergies
  • List of current medications (prescription or over-the-counter) including:
    • Name
    • Dosage
    • Frequency
  • Related lab/blood work within the last 6 months
  • Related X Rays
  • Records from the referring physician

Also, be prepared to share your medical history with our physicians, including any medical conditions you have or may have had in the past.
